Early Learners Program (Infants to Pre-K)
Forms are listen in alphabetical order:
- Child Health Report
- Update it annually for ages 24 months and above; otherwise, updated every 6 months for ages 23 months or younger
- Early Learners Program Policies 2026-2027
- Sign and return the last page and update it every 6 months
- Emergency Contact Form
- Update every 6 months or when changes occur
- Photo & Media Acknowledgement
Elementary & Middle School Program (K to 8th)
Forms are listen in alphabetical order:
- Child Health Report
- Required for all new students + entry into Kindergarten and Sixth Grade
- Students in Before and After Care (child care) must submit a physical every 12 months
- VCA Elementary/Middle School Policies & Handbook
- Complete this portion and sign online
- Dental Form
- Required for all new students + entry into Kindergarten, Third, and Seventh Grades
- Photo & Media Acknowledgement
- New Student Application
- Wellness Policy
