Early Learners Program  (Infants to Pre-K)

Forms are listen in alphabetical order:

    • Sign and return the last page and update it every 6 months
  • Child Health Report
    • Update it annually for ages 24 months and above; otherwise, updated every 6 months for ages 23 months or younger

Elementary & Middle School Program (K to 8th)

  • Child Health Report
    • Required for all new students + entry into Kindergarten and Sixth Grade
    • Students in Before and After Care (child care) must submit a physical every 12 months
